Nutrition Consultant – Head Start

Summary of Duties:
Whereas the Agency operates Head Start, Early Head Start, and GSRP programs in several Michigan counties and is in need of the services of a professional consultant dietitian in the field of pediatrics; and whereas the Consultant is a firm able to provide a professional pediatric consultant dietitian and desires to render services as an independent contractor on behalf of the Agency and whereas the parties desire to set forth their respective rights, duties, and obligations to each other; now, therefore, in consideration of the mutual promises and covenants contained herein, the parties agree as follow:

Services:
 Assess the nutritional status and special needs of children and their families (referred by FSC, Teaching staff, Area Managers or Nutrition Questionnaire Screen and/or parental request) from information provided by the family and from health records; discussions with Agency staff and health professionals. Prioritization will be as follows:
Parent Request
 Nutrition Questionnaire (NQ) with 3 or more risk factors identified by staff
 Obesity (as defined by CDC equal or greater than 95th percentile )
Underweight (as defined by CDC — less than 5th percentile)
Provide nutrition counseling/information to parents;
Provide support/assistance to staff/families in interpreting nutritional assessment data; including information on specific dietary health issues;
 Function as liaison, when necessary and appropriate, between Agency staff, families, and health professionals for children with special nutritional/health needs;
Provide a nutrition education program to parents on topics such as, but not inclusive of, the importance of healthy eating, and the selection and preparation of nutritious foods that meet the family’s nutritional and food budget needs (HSPPS 1302.46(b)(10;
Provide, upon request, nutrition-related health education to staff;
Attend two Health Services Advisory Committee (HSAC) meetings during tenure of the contract. (1302.42(b)(4))
Provide any additional consultation as deemed necessary
 Document follow-up with families and staff in ChildPlus database.
 Maintain communication with Head Start Health Services Content Specialist and other staff as necessary
 
Certificates, Licenses, Registrations:
Registered dietitian, registered dietitian nutritionist; nutritionist with relevant qualifications; or Bachelors or Master’s degree in relevant discipline; and at least two years of professional level experience in health Services, preferably in a social service environment is required. Must have working knowledge of computers and a reliable internet connection. 

Additional Information: 
Consultant’s relationship with the Agency will be that of an independent contractor Working approximately 16 hours per week; Reimbursement is as follows: $49.00 per hour non-travel consulting rate; includes preparation time for parent/staff education/training; $66.00 per travel consulting hour (covers mileage)
